Pre-surgical evaluation: Including pre- bariatric and spinal cord stimulator procedure. 

Child/Adult Assessments: Behavioral and cognitive/developmental evaluations to assess for ADHD, learning disorder, Autism Spectrum Disorder, Intellectual/Developmental Delays, Guardianship, and more. Children as young as 18 months can be assessed for Autism and developmental delays. 

Addiction risk assessments: Prescribing physicians may require an addiction risk assessment prior to the prescription of controlled substances. 

Business Consultation: Whether you are an individual clinician, a group practice, or agency, I can provide training and consultation on various kinds of assessment. These can be done virtually and in person. 

Rates

Evaluations/Assessments:

Pricing depends on the type and scope of the evaluation. They can range from $200 to $3000. This accounts for the initial clinical interview, evaluation, scoring, record review, interpretation, report writing, and feedback session. I currently accept some health insurance plans, but I do offer sliding scale rates, so please contact me to discuss.

Consulting session:

This service is designed for individual clinicians or practices that would like training/consulting on conducting various kinds of assessments. The hourly rate for consulting is $150+ depending on location.
